How Far Along to Find Out Gender: The Real Timeline, and How to Explain It

Clinical cell-free DNA screening can report fetal sex chromosomes from a maternal blood draw at about 9 to 10 weeks of pregnancy, according to guidance from the Society for Maternal-Fetal Medicine and the manufacturer instructions for the assays themselves. Ultrasound becomes dependable later: a Czech study of 1,222 pregnancies found the genital area could be assessed in 97.4% of fetuses once crown-rump length reached 60 mm, around 12 weeks 2 days, while the routine anatomy scan is performed between 18 and 22 weeks in the United States. None of these tests reports gender. They report sex chromosomes or the appearance of external genital anatomy, which is a narrower and more literal thing.

The question is really two questions, and only one of them has a test

Search results, product packaging and gender-reveal marketing all use one word for two different objects. A laboratory analyzing cell-free DNA in a maternal blood sample is counting chromosome fragments and looking for Y-chromosome sequences. A sonographer at 20 weeks is looking at the shape of external genitalia through an abdominal probe. Gender identity, the sense a person eventually has of themselves, is not a chromosome count and not a structure visible on a screen. No prenatal laboratory in the United States claims to measure it.

The distinction matters for a practical reason before it matters for any other. Test reports use the narrow term, and clinicians answer questions using the narrow term. A patient who asks "when will I know the gender" and receives an answer about "fetal sex" has not been corrected on a technicality; they have been given the only measurement that exists. The Society for Maternal-Fetal Medicine, in Consult Series #74 published in 2025 and endorsed by the American College of Obstetricians and Gynecologists, refers throughout to sex chromosome screening and sex chromosome differences, never to gender.

What each method can report, and from which week

Six routes exist, and they differ in what they physically observe.

Two columns in that table carry most of the weight. "Kind of test" separates a screening result, which changes a probability, from a diagnostic result, which identifies chromosomes directly. "Turnaround" separates the date blood leaves the arm from the date an answer arrives, which is the gap that catches people planning an announcement around a specific weekend.

How early a blood screen can report sex chromosomes

ACOG and SMFM Practice Bulletin 226, the standing guidance until it was replaced in late 2025, stated that cell-free DNA screening "can be performed at any time after 9-10 weeks of gestation." Natera's patient information for Panorama says the test "can be performed as early as nine weeks gestation" and that "most results will be returned to your doctor within 5-7 calendar days from the date that samples are received." A hospital send-out listing from University of Michigan MLabs gives an analytic time of 6 to 8 days for the same test, so the practical window from draw to phone call is closer to a week and a half than to five days.

The floor at 9 to 10 weeks is biological. The screen needs enough placental DNA in the sample, expressed as fetal fraction, and most laboratories treat a fetal fraction below 4% as too low to report. That threshold is not universal. Some standard operating procedures accept 3.5%, and detection limits for trisomy 21 have been placed as low as 2% at sufficient sequencing depth, so two laboratories can return different verdicts on the identical sample.

Accuracy for the sex-chromosome question is high and well documented. A bivariate meta-analysis by Mackie and colleagues, published in BJOG in 2017, pooled 11,179 tests and found sensitivity of 98.9% and specificity of 99.6% for fetal sex. The literature disagrees about whether timing degrades that performance. Devaney and colleagues, writing in JAMA in 2011, reported sensitivity of 74.5% before 7 weeks, rising to 94.8% between 7 and 12 weeks and 99.0% after 20 weeks. Wright, Wei, Higgins and Sagoo, reviewing 90 studies and 10,587 fetal sex results in BMC Research Notes in 2012, concluded the opposite, finding that results "vary very little with trimester or week of testing."

A second point gets flattened in most coverage. NIPT is not a sex test with a chromosome screen attached. It is a screen for trisomies 21, 18 and 13, where Gil and colleagues reported a detection rate of 99.7% and a false-positive rate of 0.04% in Ultrasound in Obstetrics & Gynecology in 2017. Sex chromosome reporting is an option layered on top, and SMFM's 2025 guidance states that "screening for sex chromosome differences should be optional," citing higher false-positive rates and the risk of detecting a chromosome difference in the pregnant person rather than the fetus.

What ultrasound can see, and when it can see it

Ultrasound reports anatomy, so its limits are the limits of a picture. Lubusky and colleagues at University Hospital Olomouc examined 1,222 singleton pregnancies at 12 to 14 weeks and published the feasibility figures separately from accuracy in Biomedical Papers in 2012. Below a crown-rump length of 50 mm, under about 11 weeks 4 days, the genital area could be assessed in 39.1% of cases and the assessment was correct 30.5% of the time. At 55 to 59.9 mm the figures were 90.5% feasibility and 96.6% accuracy. At 60 mm and above, roughly 12 weeks 2 days, feasibility reached 97.4% and accuracy reached 100% for both sexes.

Efrat and colleagues found a similar pattern in 656 pregnancies reported in Ultrasound in Obstetrics & Gynecology in 2006. An assignment was possible in 555 of them, about 85%, because an intermediate genital tubercle angle of 10 to 30 degrees was recorded as undetermined rather than guessed. Among the assignments made, 99.6% of males and 97.4% of females were correct.

The anatomy scan is not a sex-determination appointment that happens to check organs. ACOG places the second-trimester scan for fetal structural defects "ideally" between 18 and 22 weeks. The UK's NHS Fetal Anomaly Screening Programme sets its window at 18 weeks 0 days to 20 weeks 6 days and states plainly that finding out the sex of the baby is not offered as part of the national screening programme, leaving disclosure to individual hospital policy. Norfolk and Norwich University Hospitals guidance goes further and says a further scan will not be offered to determine fetal sex if it was not established during the anomaly scan. Crossed legs, a cord between the thighs, an anterior placenta or maternal body habitus can all end the question for that day.

At-home DNA kits and clinical NIPT are different products

Both analyze DNA from a blood sample, which is where the resemblance stops.

SneakPeek, the most visible at-home product, states its test can be used "as early as six weeks into pregnancy" and reports results the same day or the next day after the laboratory receives the sample. Its headline accuracy figure comes from a 2020 study in the International Journal of Pregnancy & Child Birth: 1,029 pregnant women between 7 and 37 weeks, of whom 1,028 results matched the child's sex at birth, with Y-chromosome DNA detected in 505 of 506 samples from mothers who delivered boys. The authors were company-affiliated, and the six-week claim rests on a smaller 2022 study of 103 women confirmed largely against the company's own later samples rather than against birth outcomes. Independent peer-reviewed validation by a third party does not appear in the published record.

The kit's own terms describe the products as "for informational and educational use only and not intended to diagnose or treat disease." The failure mode manufacturers warn about is contamination: instructions require washing and air-drying hands and cleaning the arm with a supplied brush, because male DNA from any source in the household can produce a male result.

Federal regulators have addressed the wider category. On 19 April 2022 the FDA issued a safety communication stating that no non-invasive prenatal screening test has been authorized, cleared or approved by the agency. Jeff Shuren, M.D., J.D., then director of the FDA's Center for Devices and Radiological Health, said: "While genetic non-invasive prenatal screening tests are widely used today, these tests have not been reviewed by the FDA and may be making claims about their performance and use that are not based on sound science." He added that "without proper understanding of how these tests should be used, people may make inappropriate health care decisions regarding their pregnancy."

When a result is uncertain, or two tests disagree

Samples fail. Reported no-call rates across clinical protocols and sequencing platforms range from 0.1% to 6.1%. In a cohort of 242,607 SNP-based tests analyzed by Benn, Martin, McKanna, Valenti, Billings and Demko in the Journal of Genetic Counseling in 2020, a redraw was recommended for low fetal fraction or low-confidence data in 3.55% of all cases. Of those redraws, 63.3% returned an informative result, and the authors modeled that roughly 0.7% of patients would still be without an answer after two attempts. A low fetal fraction is itself information rather than an equipment fault, since the same paper found 48% of women with fetal fraction below 4% fell into an elevated-risk category on the authors' algorithm.

Discordance between a blood screen and a later scan is uncommon and documented. Reported rates of fetal sex disagreement between NIPT and anatomical ultrasound range from 0.0% to 0.9%, roughly 1 in 1,500 to 2,000 pregnancies. Causes include sample mislabeling, a vanishing twin whose cell-free DNA can persist for eight weeks or more after demise, confined placental mosaicism, a sex chromosome difference in the pregnant person, and differences of sex development. The TRIDENT-2 study found no discordant-positive results when testing was performed after 15 weeks.

A screening statistic never converts into a diagnostic guarantee, and sex chromosome findings show why. A 2023 systematic review of 94 studies covering 1,531,240 screened women found pooled sensitivity of 94.1% and specificity of 99.5% for sex chromosome aneuploidies. Positive predictive value tells a different story: an analysis of 67,099 pregnancies put the overall confirmed rate for flagged sex chromosome findings at 38.26%, and monosomy X performs worst, with published estimates near 14.5%. Natera's own patient material states that "Panorama is a screening test, which means that this test does not make a final diagnosis" and that "high risk test results need diagnostic confirmation by alternative testing methods."

How to explain a result to someone else

Most confusion at a baby shower comes from a sentence that omits the method. Three moves fix it.

  1. Name the method and the week. "A blood screen at 11 weeks" and "the anatomy scan at 19 weeks" carry different weight, and the listener cannot weigh a result whose origin is hidden.
  2. Say what the method observed. A blood screen detected or did not detect Y-chromosome sequences. A sonographer saw genital anatomy at a given crown-rump length. Both statements are literally true and neither claims more than it measured.
  3. Say what confirms it and when. A screening result is confirmed by a diagnostic test or, in practice for most families, corroborated by the anatomy scan between 18 and 22 weeks.
